lifeCHARTS Emergency Medical Card or EMC is card that holds the credentials to log onto a secure online personal health record with. This record is personally managed, and may contain unchangeable clinical documents that have been attached via requests to lifeCHARTS MMS clinics or documents that have simply been scanned in. All clinics who use lifeCHARTS MMS will have some portion of patient chart data accessible by the patients themselves via their EMC login. Individuals who wish to purchase an EMC card may do so and be independent of any lifeCHARTS affiliated clinic.
